Cancellation & Refund Policy

United Mobile Phlebotomy provides mobile medical services that require travel time, scheduling allocation, and provider dispatch. By booking and submitting payment, you agree to the following terms:

1. Appointment Confirmation

Appointments are confirmed only upon receipt of payment. Payment secures the scheduled visit and reserves provider time and travel allocation.

2. Cancellations (24 Hours or More)

Cancellations made at least 24 hours prior to the scheduled appointment are subject to an administrative fee. Any remaining balance will be eligible for reembolso accordingly.

3. Late Cancellations (Within 4 Hours)

Cancellations made within 4 hours of the scheduled appointment time are not eligible for reembolso.

4. No-Show Policy

A no-show includes, but is not limited to:

  • Patient not present at the scheduled location

  • Incorrect or incomplete address provided

  • Failure to answer calls/texts upon arrival

  • Refusal of service at time of visit

  • Failure to provide required documentation (ID, lab order, etc.)

In such cases, the appointment is considered fulfilled and not eligible for reembolso.

5. Patient Preparation Responsibility

Patients are responsible for reviewing and following all required instructions prior to the provider’s arrival, including laboratory-specific guidelines and kit readiness (when applicable).

If the patient is not properly prepared at the time of arrival, the visit will be considered completed and charges will apply.

6. Provider En Route / Arrival

Once the provider has been dispatched or is en route to the appointment location, the visit is considered in progress. Cancellations at this stage are not eligible for reembolso.

7. Laboratory Kit Services (Rupa, Vibrant, etc.)

Patients must ensure:

  • Test kit is present

  • All paperwork is completed

  • Proper preparation instructions have been followed

United Mobile Phlebotomy is not responsible for laboratory errors, shipping delays, or kit deficiencies.

8. Re-Scheduling

One rescheduling request may be permitted with at least 24 hours notice. Additional rescheduling requests may be subject to administrative fees.

9. Safety & Right to Refuse Service

United Mobile Phlebotomy reserves the right to refuse or discontinue service if:

  • The environment is unsafe

  • There is aggressive or inappropriate behavior

  • Required conditions for safe specimen collection are not met

In such cases, the visit will be considered fulfilled.

10. Weather & Unforeseen Circumstances

In the event of severe weather or circumstances beyond our control, appointments may be rescheduled without penalty at the discretion of the company.

11. Payment Disputes

By completing payment, the client acknowledges acceptance of this policy. Any payment disputes or chargebacks made in contradiction to this policy may be subject to documentation review and administrative handling.
